Spay/Neuter Contract

Description of the Program:

Every year in the United States, millions of homeless animals are killed in animal control facilities. Getting animals Spayed/Neutered is one of the most powerful avenues available to combat this tragedy. If you need assistance getting your dog or cat altered, CCLS can help finance the surgery. CCLS works with Lakewood Veterinary Services in Rushford, NY to alter animals. In addition to altering, CCLS will also help finance other medical care an animal may need, (such as tumor removal, treatment for an infection, etc.) These types of appointments will be made with a veterinarian of CCLS’s choice.

In order to make this program run smoothly, we ask that you comply with the guidelines below. By initialing each guideline and signing the document, you are agreeing to the policies set forth by CCLS in this contract.

1. I am the legal guardian of this pet. This pet does not belong to anyone else but my family and me.*

2. As the pet’s guardian who has the pet’s best interest at heart, I will not sell or give away this animal. This animal is a family member.*

3. If I need to cancel an appointment, I will let CCLS know at least two days in advance. (Exceptions may be made for emergency situations).*

4. In some cases, it may be easier for CCLS to meet me and my pet at the veterinarian’s office. In other cases, it may be easier for CCLS to pick the animal up at my home. I will work with CCLS to come up the with a mutually agreed upon plan.*

5. CCLS will not spay a dog or cat who is in heat; an appointment will be made at a later date when the animal is no longer in heat.*

6. If, in the opinion of the veterinarian, altering cannot be done safely, CCLS will not go through with the surgery.*

7. CCLS will not spay pregnant animals at any stage of pregnancy. If there is a chance an animal may be pregnant, she will not be spayed until after she has her babies or is confirmed not to be pregnant.*

8. CCLS will finance as little or as much of the surgery as the guardian needs. Because CCLS is funding the animal’s care, any recommendations for the animal’s well-being (pre-surgical bloodwork, pain medicine after surgery, etc.) will be followed.*

9. If there are any complications with a surgery, CCLS will make every attempt to notify the guardian as soon as possible.*

10. If a guardian has any concerns about their pet receiving a particular medicine, getting a particular procedure done, etc., any and all concerns must be brought to the attention of CCLS before the appointment. CCLS will not have anything done to an animal that the guardian has specifically requested not to.*

11. If the veterinarian requests a follow-up appointment(s) with a pet, the guardian agrees to work with CCLS to comply with the veterinarian’s request. Follow-up appointments cannot be ignored or made with a different veterinarian. CCLS will fully fund follow-up appointments. However, if a guardian is able to help CCLS fund a follow-up appointment, it is greatly appreciated.

12. CCLS will not be held accountable for any complications (including death) that can occur during the surgery. Serious complications are extremely rare, but the guardian acknowledges that there are no guarantees regarding the outcome of the appointment/surgery and agrees to hold CCLS blameless.*
